First, download the veterinary consent form from the links provided on the website or email me and I will forward on a copy.

Fill in the owner and animal details, as well as the details of your veterinary clinic.

Contact your registered surgery and provide them with the form to gain consent from your Vet.

Once you've completed the form, return it to Spring Meadow Vet Physio via email at springmeadowvetphysio@outlook.com

Detail any preferences for your session/visit, and we may call you if we require more details.

Finally, Spring Meadow Vet Physio will contact you to confirm a time and date for your first session. The first session usually lasts between 60 minutes - 90 mins, as it includes a full assessment and treatment.
